Paternity Affidavits
In order for the father’s name to appear on your child’s birth certificate at the time of birth, a Paternity Acknowledgement Affidavit Form must be completed and signed by both parents at your local health department.

You will need to bring one form of current U.S. government issued ID for the parent and child.
